If you wish to proceed with any of our ADR services, please share the names of the Parties, their respective officers and representatives, the location of any pending case and its civil action number. Please attach or include any contract language agreed to by the Parties which specifies an ADR process.

Please read and agree to our Arbitration Rules. The parties may modify or supplement our Arbitration Rules in writing, signed by the parties and agreed to by the arbitrator.
